Instructions for providing reference list entries can be found in chapter 9. Section 9.4 tells you the four elements of a reference: author, date, title, and source., with an illustration of where to find the information for a journal article. Table 9.1 tells how to create a reference when information is missing, and section 9.5 addresses punctuation within reference list entries. The rest of the chapter elaborates each element of a reference, defining it, formatting it, and any special considerations.
You can find examples of reference entries in chapter 10. They are grouped according to their media (text, software, AV, and online media) and refer you back to the relevant sections of chapter 9 for more information.
